Youth-Adult Relationships Study, Screening Survey
 
 
We want to learn about how teenagers form relationships with important adults who are not their parents, what those relationships are like, how they influence teens, and what kinds of places (e.g., after-school programs) support positive youth-adult relationships.

As part of our study, we would like to ask you to take this 15-minute survey that asks you questions about you, your life, school experiences, and your relationships with adults. You may skip any questions on the survey that make you uncomfortable or that you do not want to answer and you may stop taking the survey at any time.

Approximately 150 teenagers from the Charlottesville/Albemarle area will take this survey and a limited number (40) will be selected for follow-up in the second phase of the study. The second phase will last three years and include two interviews per year. If you are selected for follow-up you will receive a phone call from either Valerie or Nancy (info below). Participation in the survey does not guarantee or mean you have to participate in the second phase. Even if you are not selected, your survey responses may be used in combination with other teens’ survey responses to provide information about youths’ relationships with adults in general.

If you participate in the study, you may be uncomfortable if you don’t like some of the questions we ask on the survey. You can skip any questions that make you uncomfortable and you can stop taking the survey at any time. If you participate in this study, there won’t be any benefit to you. The study may help us understand the role of supportive adults in the lives of teenagers and how we can help places such as after-school programs and schools better support positive youth-adult relationships.

The information that you give to us during this study will be kept private. Your survey information will be assigned a code number. Your name will not be used, and the list linking the code number assigned to you will be destroyed after all the data is collected. People who read our study will not know that it was you. We keep things locked up so that only our researchers see them. The only time we will tell someone what you said is if you tell us something that makes us think you might be in danger. In that case we would have to tell your parent or another adult.

You don’t have to participate in this study.You can stop doing the study at any time. If you want to stop participating in the study simply close the internet window. If you choose to stop before we are finished, any answers you already gave will be destroyed. There is no penalty for stopping. If you decide that you don’t want your survey in the study but you already turned it in, contact Nancy at 434-924-0815 or Valerie at 434-243-3786.
