CONSENT FORM
A study of assessment literacy of physical education pre-service teachers
Please read this consent agreement carefully before you decide to participate in this study.
Purpose of the research study: The purpose of the study is to explore the assessment literacy of physical education pre-service teachers.
What you will do in the study: If you give consent by clicking “I accept” at the end of the consent form, you will be asked to complete a questionnaire as soon as possible (by April 30, 2015). You may stop the questionnaire at anytime.
Time required: You will spend about 25 minutes to complete the questionnaire.
Risks: There are no anticipated risks in this study.
Benefits: There are no direct benefits to you for participating in this research study. The study may help us understand the current state of physical education pre-service teacher’s assessment literacy. This may help to make improvements for future physical education major students.
Confidentiality: The information that you provide in this study will be handled confidentially. Your information will be assigned a code number. Your name will not be used in any report, and your data will be reported in a way that will not identify you.
Voluntary participation: Your participation in the study is completely voluntary. All data from you including the questionnaire will be destroyed if you choose to withdraw from the study.
Right to withdraw from the study: You have the right to withdraw from the study at any time without penalty. If you wish to withdraw do not complete the questionnaire. If you choose to withdraw from the study after you have completed the questionnaire, you can simply inform the researcher you wish to withdraw and she will remove your data from any analyses.
Payment: You will receive no payment for participating in the study. There will, however, be prize drawings. Those that complete the questionnaire by the deadline will be entered into a prize drawing. The odds of winning depend on the number of student teachers that complete the questionnaire. There are a possible 150 student teachers that will be requested to participate in the study. Winners will be contacted via email and prizes will be mailed to the address provided by the winners.
There will be three winners: two $25 visa cards and one $50 visa card.
