Dear Touchpoints™ Training Participant:

Children’s Services Council (CSC) of Palm Beach County is very excited that you were able to attend the Touchpoints™ Individual Level Training that was held October 23-25, 2007. We sincerely appreciate all the effort and hard work you contributed during the training.

As you know, CSC is committed to doing all they can to continuously improve their training efforts and the improvement in service provision that results from participation in training. For this reason, Dr. Reid Bates has been retained by CSC to undertake the Touchpoints™ Training Assessment Initiative. As a part of this project, Dr. Bates has collected a variety of data from you. We would like to ask you to complete one final survey instrument.

The following survey asks you to reflect on the Touchpoints™ Training you attended in October and to think about how often you have used some of the things you learned in that training. The survey asks you to read a series of statements describing various behaviors and, for each statement, to make two ratings: a NOW rating and a BEFORE rating.

• The NOW rating asks you to rate how frequently you currently use the behavior in your interactions with your clients.
• The BEFORE rating asks you to rate how frequently you used the behavior in your interactions with clients before you attended the Touchpoints™ Training program in October.

The data collected will be analyzed, summarized and included in a report to report to CSC. Dr. Bates will be the only recipient of the data and will hold all data in the strictest confidentiality. No one other than him will see the responses of individual employees, and no information will be provided in reports that will identify individual respondents or in any way allow individuals to be identified.
