This survey is for individuals with disabilities and/or their caregivers, and the person with disabilities is enrolled in Medicaid. It is being conducted by the Children's Disability Rights Education Association.

Our goal is to help individuals with disabilities to access the home supports they are guaranteed under federal civil rights legislation. To do this most effectively, we need to get a better idea of how individuals with disabilities are being treated within the existing system.

There are two surveys: this one is for individuals or people who are caregiving for individuals with disabilities who are not covered by Medicaid.

Your participation in this study is completely voluntary. If you feel uncomfortable answering any questions, you can withdraw from the survey at any point. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
