HPGM AWARDS
2014 Nomination Form

Please take a few minutes and submit a nomination for the 2014 HPGM Awards. These awards recognize Hispanic leaders and the organizations that support them. They are designed to honor individuals and organizations who have exhibited a high level of leadership, achievement and responsibility; who have demonstrated an attitude of excellence in both business and community; and who should be examples for the broader community to emulate.

Criteria for Eligibility: Individual

• Be of Hispanic heritage
• Have a Bachelor’s Degree or higher
• Have not yet received this award
• Reside in the state of Wisconsin
