documents need to be submitted within 30 days by secure email to

:
1) One billing record in the past three months for Therapeutic Consultation behavioral
services (or a copy of a current, active contract with a behavioral interventionist).
2) Personnel records comfirming QDDP on staff with at least three years in providing supports
to individuals with dual diagnosis or behavioral support needs.
3) Description of the method used to establish DSP behavioral competencies and a copy of
the most recent evaluation.
4) Description of supervision process to include standard content related to the behavioral
techniques covered.
5) Agendas and sign-in sheets from last four internal trainings related to behavioral support.
6) Summary notes for last three monthly individual review meetings.
Once your documents are reviewed, your designation (if confirmed) will be posted on the
DBHDS website. DBHDS will contact you if additional documentation is needed.
