This is a survey being sent to you about the pilot electronic student learning portfolio (e-SLP). Regardless of whether you had input or not, with this pilot program please complete this survey as each of your opinions is important to us. Please be as specific as possible when answering the following questions in CPD Phase1b. It will take approximately 10 minutes to complete the questionnaire.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
