Environmental Health Designees:
You are invited to participate in a survey of EHD preparedness and support. The survey will take approximately 15 minutes to complete and has been sent on behalf of Sharon Mallory in the Vermont Department of Health as part of the requirements for a Masters in Public Health. Responses are anonymous.
Your participation in this study is voluntary, but is much appreciated. Please complete the survey by FRIDAY MARCH 25, 2005. If you have questions at any time about the survey or the procedures, you may contact me at 802-863-7548 or by email at [email protected].
This questionnaire is designed for primary and back-up EHDs. District Directors and PHN Supervisors should not take this survey unless they are active in the EHD role. I will provide DDs/PHNSs with opportunity to comment on the program in the near future
