1. Do you think you were provided adequate information about the BSW Field Education Program's expectations?

2. Did the student present in a professional manner for the interview?

3. Has the student been a "good fit" for your agency?

4. Were you made aware or told where to find student deadlines?

5. Did you know who to contact if a conflict arose with a student?

6. Were you notified of the student's start time and date in sufficient time to plan?

7. Do you think the Director of Field Education (Amanda Keys) had sufficient knowledge of the student to be helpful in working with the student?

8. If you had a problem with a student, was the Director of Field Education helpful in resolving the concerns?

9. If you attempted to contact the Director of Field Education, did they return your call/email in a timely manner?

10. Based on your experience with the SBU BSW Program this semester, would you be willing to be a Field Instructor again?
