Initial Survey_CHEM 326

Dear Participant,
You are invited to participate in this survey, which is part of a project geared towards investigating the relationship between knowledge transfer in chemistry and student attitudes towards chemistry. This survey consists of demographic questions as well as questions about your attitude towards chemistry. It should take about 5 minutes to complete this questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project, and your grade in CHEM 326 will be in no way affected by you choosing to participate or not to participate. If you feel uncomfortable answering any of the questions, you can withdraw from the survey at any point. Your survey responses will be strictly confidential and any personal information provided will not be linked to you in any way. Your information will be coded and will remain confidential as only the researcher will have access to this information. This project has been approved by the Office of Research/Human Subjects Committee at South Dakota State University and the approval number is: IRB-1406007-EXM.
