Dear Participant,

We are graduate students from the Auerbach School of Occupational Therapy at Spalding University. We are conducting a research project as part of our graduate curriculum using Web-Based Training Modules.

The goal of our research project is to investigate the effectiveness of web-based training modules and the use of technology to provide educational information to families who have children with disabilities. We appreciate your participation and ask that you take the time to complete this questionnaire. The questionnaire should take approximately 5 minutes to complete. You may decline to answer any questions.

The results from your participation will be anonymous, the information will be coded and remain confidential. There are no known risks to completing the questionnaire; however, an emotional response may be elicited to some of the questions if a participant were sensitive to the topic of those items.
