Survey of Experience with Virtual Environments
Thank you again for agreeing to take part in this research project. As outlined in the Research Project Information Sheet, this is the pre-participation survey and it will take a maximum of ten minutes.
David Holloway, a PhD student at University of the Sunshine Coast, is conducting research that evaluates the usefulness of a virtual environment simulation experience in regard to safe administration of medication. To assist this research we would like to collect data about your views about your previous experience in the use of virtual environments and 3D games. Participation in this research is VOLUNTARY. Your responses will be entirely CONFIDENTIAL and ANONYMOUS. No identifying information will be collected. The data gathered will be used only in relation to the project and and is not related to your subject choices as part of your nursing degree. Non participation will not impact on your study. The data gathered will be aggregated and you will be entirely unidentifiable. The results will be included in the project report and may be included in publications and conference presentations
Your decision to continue with this survey will be considered as having provided informed consent. |

* How much time each week do you spend playing games on a computer, games console or tablet/phone? |
| |
* Of the time you spend playing games, how much of that involves playing 3D games? 3D games include any game where you can move in all three dimensions as one would in real life. Common examples include: Minecraft, Call of Duty, Halo, Tomb Raider, Grand Theft Auto |

* What is the device you spend the most time playing on? |
| |
* When I choose the gender of a character / avatar I’m going to play in a game, I tend to choose: |

| * What are the most recent game titles you’ve played on any device? (List up to 5 – if you’ve played more than 5, list the most frequently played five) | | |

* Which of the following terms would you apply to your experience playing 3D video games? |
| |

* Circle the statement that you think best applies to your thoughts on your use (or non-use) of video games: |

* Aside from games, have you ever used any other 3D virtual environments for social purposes. Examples include: Second Life, IMVU, 3D Facebook or website games where you control / style / dress up an avatar. This can include 3D social games you may have played as a child or teen. |

* 3D virtual environments for nursing simulation are more gimmick than a serious tool |
| |
* Overall, 3D virtual environments or games are a positive addition to society |
| |
* A well-constructed 3D virtual environment could provide a realistic nursing scenario that helps me as much as a ‘real-life’ simulation or practicum experience |
| |
* I believe a well-constructed 3D virtual environment could directly impact my competence in performing a task like medication administration |
| |
| * Final question: please enter the participant number you were provided at the beginning of this session. This will in no way identify you - it is purely used to link your survey responses to your performance in the simulation. | | |
