Dear Survey Participant,

I am currently enrolled in a graduate program at Tusculum College that requires the submittal of a research project for graduation. In order to complete my project, I must conduct a survey as a method of gathering data. My survey is designed to gather data on your perception and attitudes towards distance education. Your participation in the completion of this survey is completely voluntary.

Distance education has shown a tremendous growth over the years. It provides an alternative education method to be able to complete a course anytime, any place, and anywhere. The results of this survey may determine if there is a need to offer more types of distance education courses in Tennessee higher education facilities.

This survey consists of 15 questions and should take approximately 5 minutes of your time. The participants of this survey will remain anonymous. Therefore, I ask that you do not place your name on this survey and answer it completely honesty. For you convenience and anonymity, I have designed this form electronically.
