If you do not attend Ward Melville, please do not complete the survey, as it may skew our results. For Ward Melville students, this survey is intended to chart a variety of your preferences, providing us with information about what clubs or activities should potentially be instated.

We do not ask you for any identifying information of any kind, and we have no way of obtaining any information that you do not expressly give us from this survey. These results will not be used for any publications, and are NOT ATTACHED TO YOUR NAME, or any other information that could identify you. Please answer truthfully; false information will give us incorrect results. Every question is important, although you are not required to continue taking the survey if you do not wish to.
