Thank you for taking part in my survey. I aim to gain better knowlegde of the support and preparation children have when they come into hospital planned or unplanned so I can take this into account when producing the children's pack.
However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point or leave it blank.
