Before you begin, have the following information readily available:

Compensation methods, rates and average annual income ranges for drivers and non-drivers.
Contributory and voluntary benefit details for medical, pharmacy, dental, 401k and paid time off.
For all medical and dental plans offered: deductibles, co-insurance, out-of-pocket maximums, co-pays and employee contributions.
Wellness or health improvement programs offered.

The survey will take 15-20 minutes to complete.
