DBMD 1: Personal Data Inventory - Part One

This inventory is designed to introduce you to your District Board of Ministerial Development (DBMD) as you apply for district ministerial standing. You are expected to fill out the inventory frankly and carefully in order that the DBMD might better know you and your background, and thereby assist you in your progress toward earning your ministerial credentials. You are responsible to answer all applicable questions.

Some questions are simply Yes/No answers but allow for comments. Comments are not always necessary for these questions, but if you feel you should add further clarification or detail, or if the question specifically requests further explanation, please use the provided comment section.
